Transition Services exists to empower people to enhance their quality of life through support and accountability around self-determined goals.

Transition Services staff help our community members navigate complex social services systems and advocate on their behalf. Our trained case workers help with problem solving and provide support in overcoming a variety of barriers.

This support looks different for each person, but often includes practical assistance with getting ID, accessing financial benefits, or connecting to resources like legal help, doctors and mental health or addictions treatment.

A primary goal for each participant in the Transition Services Program is to find and maintain housing. Our staff help with finding housing, in home supports for preventing eviction, and assist our clients in having positive relationships with their landlords.
